Ordering Note: Approval by the Laboratory Medicine Resident (LMR) on-call is required for all orders. Molecular testing does not replace bacterial or fungal cultures. The Biofire Meningitis/Encephalitis has demonstrated false positive and false negative results. See the following preferred tests for specific agents: HSV, VZV, Cryptococcus, bacteria. Other specific tests are available for: CMV, HHV6, Enterovirus/Parechovirus, fungal culture, bacterial PCR, Cryptococcus PCR, pneumococcal urine antigen.

- Note: Clotted CSF is not acceptable for this test and will be rejected.
- Testing will only be performed on CSF from lumbar puncture. CSF shunt fluid is not an appropriate sample for this testing.
- Approval Required
- Laboratory Medicine Resident (LMR) approval is required.
- Handling Instructions
Outside Laboratories: Freeze CSF in a sterile container at -20°C.
Close coordination with courier logistics is required upon approval/sendout. Testing should not be approved unless timely transport to the performing laboratory can be achieved.
Stability: Frozen: 7 days; Refrigerated: 24 hours; Ambient: 1 hour.

Processing

Freeze CSF in a sterile container at -20°C. Notify the appropriate bench of an incoming sample or add-on order. This test should be prioritized for immediate sendout.
- UWMC: Freeze sample in Sendouts rack and notify UW-MT Sendouts.
- HMC: Freeze sample in the Micro rack and notify HMC Microbiology.

- Order SCHL Test: LAB2421.
- Interfaced: Yes. [Interface: 601; Department: SOSC]
- Send samples as received to Seattle Children's Hospital Lab via Delivery Express courier with a copy of the MayoAccess Interface Packing List.
- Samples that do not arrive at SCHL within 24 hours of collection must be frozen.
Stability: Frozen: 7 days; Refrigerated: 24 hours; Ambient: 1 hour.
