Ordering Recommendation: Aids in evaluation of patient with allergic signs and symptoms, such as anaphylaxis; may assist in diagnosing and monitoring of mast-cell activation disorders.
For evaluating patients for diseases of immediate hypersensitivity, the preferred screening test is Tryptase [TRYPT]. Test results are highly dependent on proper sample collection. Histamine may be released ex vivo by passing basophils in blood through a small-bore needle under vacuum or when blood clots. Serum should not be used in order to avoid the artifactual release of histamine from basophils that can occur during clotting.

Collect 3 mL blood in a pre-chilled Lavender (EDTA) top tube. After collection, place sample immediately on wet ice and expedite transport to the lab.

Outside Laboratories: Centrifuge sample in a refrigerated centrifuge (2-8°C) and separate the upper two-thirds of plasma within 20 minutes. Transfer 1 mL plasma to a separate plastic vial and freeze immediately at -20°C. Transport on dry ice.
Stability: After separation from cells: Frozen (Critical): 6 months; Refrigerated: 6 hours; Ambient: Unacceptable.
Unacceptable Conditions: Nonfrozen or hemolyzed specimens.
