Description

Useful For:

  • Evaluating patients with possible diagnoses of erythropoietic protoporphyria or X-linked dominant protoporphyria

  • Establishing a biochemical diagnosis of erythropoietic protoporphyria and X-linked dominant protoporphyria

Ordering Guidance: This test may be ordered directly or performed as a reflex from Porphyrins (Total), RBC (Sendout) [RWBPOR]. Do not order concurrently with the total porphyrins RBC test.

Patient Preparation: Patient should abstain from alcohol for 24 hours.

Ordering Requirements: Include a list of medications the patient is currently taking in the Epic order comment.

Reference Values:

Free Protoporphyrin: <20 mcg/dL
Zinc-complexed Protoporphyrin: <60 mcg/dL

Interpretation: Abnormal results are reported with a detailed interpretation that may include an overview of the results and their significance, a correlation to available clinical information provided with the specimen, differential diagnosis, and recommendations for additional testing when indicated and available.

Interferences and Limitations

Cautions: Patients must abstain from alcohol for at least 24 hours prior to specimen collection. Alcohol suppresses enzyme activity potentially leading to false-positive results.

Preferred: 4 mL blood in DARK GREEN (Sodium Heparin) tube placed on wet ice immediately after collection

Also Accepted (on ice): Dark green (lithium heparin), lavender (EDTA) top. Unspun Lime green PST tubes are also acceptable.

Place sample on wet ice after collection and expedite transport to the lab.

Handling Instructions

Outside Laboratories:

  • Keep sample on wet ice until it can be refrigerated. Refrigerate whole blood while awaiting transport. Ship with a cold pack to maintain proper temperature.
  • Include a list of medications that the patient is currently taking on the DLMP requisition.*
    • *Use client-specific requisition when available. To obtain a client-specific requisition, contact Client Support Services via email at: commserv@uw.edu.

Stability: Refrigerated (required): 7 days; Frozen: Unacceptable; Ambient: Unacceptable.

Reject Due To: Gross hemolysis.

Processing

Receiving Instructions

Refrigerate unspun whole blood as soon as possible. Keep sample on wet ice until it can be transferred to the refrigerator.

  • If a list of patient's current medications is included (in the Epic order comment or on the DLMP requisition), edit the MayoAccess order before closing to enter that information in the Performing Lab Notes. Alternatively, print a copy of the Epic/DLMP requisition and package it with the associated Mayo Batch List.
