Description

Useful For:

  • As an adjunct in the diagnosis of medical conditions associated with increased bone turnover
  • Monitoring effectiveness of antiresorptive therapy in patients treated for osteopenia, osteoporosis, Paget disease, or other metabolic bone disorders
  • For monitoring therapy, a baseline specimen should be collected prior to initiation of therapy. Subsequent specimens for comparison should be collected at the same time of day as the baseline specimen.

Patient Preparation:

  • For 24 hours prior to specimen collection, patient should refrain from taking multivitamins or dietary supplements containing biotin (vitamin B7), which is commonly found in hair, skin, and nail supplements and multivitamins.

Ordering Note: This test can be ordered for both random and timed urine collections.

Method

Quantitative Chemiluminescent Immunoassay (CLIA)

Reference Range

Ref. Range Notes

Reference Values:

Males:

7-9 years: 167-578 nM BCE/mM creatinine
10-12 years: 152-505 nM BCE/mM creatinine
13-15 years: 103-776 nM BCE/mM creatinine
16-17 years: 34-313 nM BCE/mM creatinine
18 years and older: 21-83 nM BCE/mMcreatinine

Females:

7-9 years: 201-626 nM BCE/mM creatinine
10-12 years: 173-728 nMBCE/mM creatinine
13-15 years: 38-515 nM BCE/mM creatinine
16-17 years: 20-144 nM BCE/mMcreatinine
Premenopausal: 17-94 nM BCE/mM creatinine
Postmenopausal: 26-124 nM BCE/mM creatinine

Interpretive Data:

NTx Units = nM BCE/mM creatinine

A decrease of 30-40% from the NTx baseline after three months of therapy is a typical response to anti-resorptive therapy.

NTx = Cross-linked N-telopeptide of Type I Collagen
BCE = Bone Collagen Equivalent

Ordering & Collection

Specimen Type
Urine
Collection

Random Collection: Collect second-morning void urine in a screw-cap, urine collection container without preservative.

Timed Collection:

  • Collect urine for 24 hours in a preservative-free amber polyethylene 24-hr urine collection container.
  • Refrigerate sample during collection.
  • Record the collection interval.
Handling Instructions

Outside Laboratories:

  • Timed Collections: Mix specimen well. Measure and record total volume.
  • Document the collection interval and total volume or indicate 'random collection' on the DLMP requisition, as appropriate.
  • Transfer 4 mL (1 mL min) urine from random collection to a screw-cap vial, leaving room for expansion at the top. Freeze at -20°C while awaiting shipment. Transport on dry ice.

Stability: Frozen (preferred): 4 weeks; Refrigerated: 1 week; Ambient: 24 hours.

Reject Due To: Specimens contaminated with blood or extensive hemolysis.

Quantity
requested: 1 mL urine
minimum: 0.5 mL urine
