Description

Useful For: Screening for mast cell activation disorders including systemic mastocytosis. 2,3-Dinor-11beta-prostaglandin F2 alpha (2,3 BPG) is elevated in the urine of patients with systemic mastocytosis (SM). When 2,3 BPG is used in combination with Leukotriene E4, 24 Hour Urine (Sendout) [RULTE4] and N Methylhistamine, 24 Hour Urine (Sendout) [RUNMHT], the sensitivity for SM detection increases to 90%.

Elevated levels of 2,3-dinor-11beta-prostaglandin F2 alpha (2,3 BPG) in urine are not specific for systemic mast cell disease and may be found in patients with angioedema, diffuse urticaria, or myeloproliferative diseases in the absence of diffuse mast cell proliferation.

Systemic mast cell disease is a heterogeneous disease, and some patients may not have elevated 2,3 BPG in urine.

Patient Preparation: Patients taking aspirin or nonsteroidal anti-inflammatory drugs (NSAID) may have decreased concentrations of prostaglandin F2 alpha. If possible, discontinue for 2 weeks or 72 hours, respectively, prior to collecting a specimen.

Ordering Requirements: Pre-approved for UW Medicine Allergy Clinics. Orders from other locations require Laboratory Medicine Resident (LMR) approval.

Collection Note: A 24-hour urine collection is the preferred specimen type, but a random specimen is also acceptable.

24 Hour Collection: Collect urine for 24 hours in a preservative-free amber polyethylene urine collection container. Refrigerate during collection (preferred). Record the collection interval.

  • The application of temperature controls (refrigeration) must occur within 4 hours of completion of the collection.
  • If the total volume provided is less than 300 mL, the order will be changed to the random urine test by Mayo Clinic.

Handling Instructions

Outside Laboratories:

  • Mix urine well. Measure and record total volume. Document the collection interval and total volume on the DLMP requisition.
  • Aliquot 5 mL urine to a plastic screw-cap vial and refrigerate specimen.

Stability: Refrigerated (Preferred): 14 days; Frozen: 30 days; Ambient: 8 hours.
