Description

Useful For: Investigation of primary aldosteronism (eg, adrenal adenoma/carcinoma and adrenal cortical hyperplasia) and secondary aldosteronism (renovascular disease, salt depletion, potassium loading, cardiac failure with ascites, pregnancy, Bartter syndrome).

Patient Preparation: The plasma renin activity cannot be interpreted if the patient is being treated with spironolactone (Aldactone). Spironolactone (Aldactone) should be discontinued for 4-6 weeks before testing.

Ordering & Collection

Specimen Type
Plasma
Collection

Prechill a 7 mL LAVENDER TOP tube. Draw blood from a patient in a seated position into the pre-chilled Lavender top tube, mix, and immediately place in an ice-water bath. Transport to the laboratory without delay.

Handling Instructions

This test cannot be added on to a sample already in the lab due to the specific collection and processing requirements. A fresh blood collection is required.

Quantity
Requested: 2 mL plasma
Minimum: 1.2 mL plasma

Processing

Receiving Instructions

Refrigerate specimen during centrifugation. Immediately transfer plasma to a separate plastic vial and freeze at -20°C.

  • If a refrigerated centrifuge is unavailable, chill the centrifuge carriers. Centrifuge specimen for 5 minutes or less, then promptly transfer plasma.

Result-at-entry:

  • RSITE (Renin Activity Specimen Site): <enter the specimen collection site, or INP if not provided.>
    • Examples: PERP (peripheral), RARM (Right Arm), LARM (Left Arm), ANTE (Antecubital), LANT (Left Antecubital), RANT (Right Antecubital).
  • RSTIM (Renin Act Stimulation Interval): <enter the stimulation interval, or INP if not provided.>
    • Examples: BASE (Baseline), or free text (10 minutes post, etc.).

Sendouts:

  • Order Mayo Test: PRA.
  • If collection is site specific, indicate the collection site under "Report Notes" in MayoAccess order.

Stability: Frozen (critical): 14 days; Refrigerated: Unacceptable; Ambient: Unacceptable.
