Test Information: Initial testing includes type and screens for the potential transplant recipient and potential donor. If the potential donor is ABO compatible with the potential recipient, then a crossmatch is performed. If donor is found ABO incompatible with recipient, a crossmatch is not performed.
Ordering Requirements: Providers should complete a Bloodworks Northwest Request for Blood and Transfusion Testing requisition for each patient.
- "Recipient" or "Donor" should be specified on each request form. Enter the corresponding donor/recipient information in the designated fields.
- Potential donor and recipient samples may be submitted to Bloodworks NW separately or at the same time. Samples should be sent out as received.

- Also Accepted: 6 mL blood in LAVENDER TOP (EDTA) tube
Unacceptable: SST or GEL tubes
Dual Signatures Required:
- At the time of collection, a two-person verification of positive patient identity must be performed in the presence of the patient by the person collecting the specimen and a second qualified staff member. This includes a verbal read back of the full legal name, spelling of the legal name, and MRN as it appears in the medical record.
- Both people performing verification must sign or initial the sample label(s).
- Write the collection date and time on the sample label.
- Signatures of both verifiers are requested on the Bloodworks Northwest form. (This is not required for testing as long as dual verification is documented on the sample label.)

Processing
- Receiving Instructions
Store and transport whole blood at ambient temperature.
- Review the sample label(s) and requisition form for accuracy. Dual verification signatures are required on the sample label(s) only.
- Package sample(s) with the associated requisition in a biohazard bag (one patient per bag). Orders received with multiple patient samples/requisitions in the same biozard bag will be rejected by Bloodworks NW.
- Send samples as received to Bloodworks Northwest via DLMP or Delivery Express courier.
- Donor and recipient samples may be sent at the same time or separately. Do not delay transport if only one is received.
- Recipient samples are held at Bloodworks for 72 hours after collection.
- Donor samples are held at Bloodworks for 14 days after collection.
