Indications: NAIT.
Test Information: Platelet antibody crossmatch testing of maternal plasma with paternal platelets by flow cytometry, platelet antigen typing of both parents’ platelets by DNA (PCR) and Class I HLA antibody identification, and PAK-Lx or MAIPA (if warranted).
Ordering Requirements: Laboratory Medicine Resident (LMR) approval and consultation with the Transfusion Medicine attending is required. To arrange for testing, contact the LMR on-call via the paging operator at (206) 598-6190.
- LMR: Prior to sample submission, contact the Bloodworks NW Platelet Immunology Lab Supervisor at (206) 689-6542 to discuss the patient's clinical history.
Ordering Information:
- Place an Epic order for RNATE under the maternal patient's MRN.
- Results are not reported under the father's record, but an order may be placed for collection. Order a LABUND (Laboratory Undefined) and specify, "20 mL EDTA Lavender Top tubes. Paternal samples to accompany maternal samples for RNATE testing."
- Complete the Bloodworks NW Platelet Immunology Laboratory Requisition with the maternal patient information. Enter the paternal demographic information (name, hospital number, date of birth, collection date) under "Father Information" on the same requisition. Completed requisitions may be uploaded to the patient chart in Epic (preferred) or accompany the patient to blood draw.
Collection Note: Limited specimen stability. Time blood collections accordingly to ensure that samples reach the performing lab within the stability limit.

Sample Collection:
- Mother: Collect all of the following:
- 20 mL blood in EDTA LAVENDER TOP tubes
- 10 mL blood in a RED TOP tube
- 10 mL blood in BLUE TOP (Citrate) tubes
- Father: Collect 20 mL in EDTA LAVENDER TOP tubes
Collection Timing: Samples should arrive at Bloodworks Northwest within 24 hours of collection.
- Avoid collections on or before holidays.
- Avoid collections on Fridays. Samples collected on Friday must arrive in the Bloodworks Northwest Platelet Immunology Lab before the 10:00 a.m. cutoff or they will be rejected.

Store transport samples at ambient temperature. Expedite transport due to short stability.
Sendouts (UW-MT/HMC) and FHCC SPS:
- Bloodworks NW Test: 3670-00
- Specimens should arrive within 24 hours from collection.
- Samples drawn on Friday must be received by Bloodworks Northwest Platelet Immunology Laboratory by 10:00 a.m.
- Package sample(s) with a copy of the Bloodworks Northwest Platelet Immunology Requisition. The BWNW requisition is required to properly identify the maternal and paternal samples for the order.
- Send samples via scheduled DLMP Courier pickups or Delivery Express according to the site’s preferred transportation method.
Stability: Ambient (required): 48 hours (preferred to arrive within 24 hours).
