Clinical Utility: ImmuKnow is an immune cell function assay that detects cell-mediated immunity in an immunosuppressed population. The assay detects cell mediated immunity by measuring the concentration of ATP from CD4 cells following stimulation.

Interpretation
- Method
Procedure: ImmuKnow technology combines cell stimulation, cell selection, and quantification of metabolic markers (ATP) to measure cell-mediated immunity. ImmuKnow measures early response to stimulation by detecting intracellular ATP synthesis in CD4 cells selected from blood by monoclonal antibody-coated magnetic beads. The amount of ATP present in stimulated blood specimens is a measure of lymphocyte activity. Since the CD4 lymphocytes orchestrate cell-mediated immunity responses through immunoregulatory signaling, the measurement of CD4 activation reflects the degree of immune function.

ATP Level Interpretation < or = 225 ng/mL Low immune response 226-524 ng/mL Moderate immune response > or = 525 ng/mL Strong immune response The ATP level ranges for ImmuKnow were established by testing 155 apparently healthy adults and 127 transplant recipients. A cumulative frequency of differences was used to select the ATP levels that give the best balance of results between immunosuppressed and non-immunosuppressed individuals. The cutoffs for the ATP level ranges are 225 and 525 ng/mL.
Sensitivity: The limit of ATP detection of ImmuKnow is 1 ng/mL.

Collection Limitations:
- Draw specimens Monday-Friday only, between 5AM and 1:00 PM. Specimens must be received in the lab by 1:30 PM for same day shipment to referral lab.
- Do not draw on holidays or on the day prior to a holiday.
- Expedite transport to the laboratory. Testing must be initiated by Eurofins Viracor within 30 hours of collection.
Collection:
- 2-3 mL blood in Sodium Heparin GREEN TOP tube.
- Unacceptable: Lithium Heparin GREEN TOP tube

Outside Laboratories: Due to the limited stability for these samples, outside facilities should arrange for testing directly with Eurofins Viracor.

Processing
- Receiving Instructions
- Maintain whole blood samples at room temperature; do not refrigerate or freeze.
- Expedite processing. Notify sendouts staff on receipt of sample in lab.
Sendouts:
- Order electronically in the Retriever system using Eurofins Viracor Test: 9000.
- Send samples Monday - Friday only the same day as collection. Samples must arrive at Eurofins Viracor for setup the next day.
- Package samples in the provided Eurofins Viracor Live Cell shipping kits. Affix the FedEx label to the outside of the box.
- A dedicated Eurofins Viracor courier will pick up packages at the UW-MT lab window at approximately 3 PM.
- Samples that miss the daily courier pickup must be sent via regular FedEx shipment.
