Ordering Recommendation: The Synovasure® Alpha Defensin ELISA test performed on synovial fluid may be used as an adjunct to aid in the diagnosis of periprosthetic joint infection (PJI). Effective April 1st, 2024, this assay no longer includes C-reactive Protein (CRP). The final result is provided as a qualitative result: Positive, Negative or Indeterminate.

Collect 1 mL Synovial Fluid in a RED TOP tube. The specimen source (eg., left knee, right shoulder, etc.) is required.
- Note: Separate specimens must be submitted when multiple tests are ordered.
- Handling Instructions
Outside Laboratories: Due to the limited stability of the sample, it is recommended that outside facilities arrange for testing directly with CD Laboratories. If submitting through University of Washington, expedite transport. Samples must arrive at the performing lab within 4 days of collection (allow for an extra day of transport from UW-MT to CD Labs).
- Indicate the aspiration site (left/right hip/knee/other) on the UW-MT requisition.
Specimen: Refrigerate fluid.
Stability: Refrigerated (preferred): 4 days; Ambient: 72 hours; Frozen: Unacceptable.
