Genetics Sendout on Cultured Cells

- Description
Test Information: Genetics/molecular sendout testing to be performed at a reference lab on cells cultured by the UWMC Cytogenetics and Genomics Laboratory. Test varies. See OLTG #599 for further details on cell culture requests.
Approval: Laboratory Medicine Resident (LMR) approval is required unless the test is specifically listed in the online test guide or ordered as Genetics Undefined Order [GENUND] or Genetics Sendout Transport Only [GTRNSP] by an approved UW Medicine Clinic.
Refer to Maternal Sample (Sendout) [RMCSND] for information on processing maternal blood samples that accompany fetal samples (CV, amniocytes) for sendout testing.

UWMC Cytogenetics:
- Once the provider confirms that sendout testing will be ordered, send a preliminary email to labfix@uw.edu to notify CAST. Include scanned copies of any requisitions that are not available in Epic.*
- *For outside clients, this would include the UW requisition and the performing lab requisition, if required.
- When the samples are ready to send out: Notify SPS, CSS, and CAST of an incoming sample via email. The ordering GC/provider and on-call Laboratory Medicine resident may also be copied.

- CAST or SPS will create an HC1 Task and respond to the email with the Task Number.
- Affix a custom 'alert' sticker to the outside of the sample bag and write in the HC1 Task Number. Include the UW DLMP requisition (or Epic order), along with the reference lab requisition (if included).
- Deliver samples to UW-MT SPS no later than 2pm for same-day shipment.
- If shipping on a Friday, confirm with Sendouts (206-598-3423) that the performing lab accepts Saturday delivery. (Note: Samples to ARUP or Mayo Laboratories may be sent via courier transport for Saturday delivery.)

Processing
- Receiving Instructions
SPS: Samples (cultured amniocytes, chorionic villi, or skin fibroblasts in T25 flasks) will be delivered to UW-MT SPS by UW Cytogenetics.
- Store and transport T25 flasks at room temperature. Do not refrigerate.
- Promptly forward the sample to an SPS Lead for accessioning.
Accessioning:
- If required, obtain LMR approval and document in the HC1 Task. Test may be pre-approved.
- Note: Resident approval must be obtained if the test is not listed in the online test guide and not ordered as Genetics Undefined Order [GENUND] or Genetics Sendout Transport Only [GTRNSP] by an approved UW Medicine Clinic.
- Samples should be considered urgent and irreplaceable. Track the progress of the order in the associated HC1 Task.
- Outside Orders: Accession manually using the associated client code. (Note that Cytogenetics assigns a U number for their own internal orders, but a U number should only be used in Sunquest for UW Medicine patients.)
- If there are any issues (missing paperwork or information), create a separate HC1 Case for CSS, reference the original task number, and attach any relevant requisitions. Notify one of the CSS Leads in lab.
- Sample must be sent out same-day as receipt in UW-MT SPS. Once accessioned, notify Sendouts staff immediately and forward the HC1 task to the Sendouts Queue.
Login: If ordered as GENUND/LABUND, enter the order in Sunquest as GSEND1. For GTRNSP orders, log as Genetics Sendout Transport Only [GTRNSP]. All other instructions apply.

Sendouts: If unable to ship same day, contact Cytogenetics Lab (206-598-4488) for overnight storage of flasks in the incubator.
- Package samples at ambient temperature with the completed sendout lab requisition or batch list.
- ARUP, Mayo, SCHL, and Bloodworks Northwest samples may be sent via courier. Samples to other laboratories should be sent directly via FedEx to ensure prompt delivery and proper tracking.
- For ARUP: Place the order in ARUPConnect and contact ARUP Client Services with the accession number to alert them of an incoming cultured sample.
- After printing the FedEx shipping label, enter the FedEx tracking number in the HC1 Task and send to the CAST queue (if sent to ARUP or Mayo, enter the Mayo or ARUP accession).
- CAST will confirm sample receipt with the performing lab the day after shipment.
- For samples received on Friday, confirm that the receiving lab accepts Saturday deliveries. If samples are unable to be received next day, return sample to Cytogenetics for storage and document the sample transfer in the HC1 Task.
