- Diagnosing congenital alpha-2 plasmin inhibitor deficiencies (rare)
- Providing a more complete assessment of disseminated intravascular coagulation, intravascular coagulation and fibrinolysis, or hyperfibrinolysis (primary fibrinolysis), when measured in conjunction with fibrinogen, fibrin D-dimer, fibrin degradation products, soluble fibrin monomer complex, and plasminogen

Normal, full-term, and premature infants may have mildly decreased levels (> or =50%) which reach adult levels < or = 90 days postnatal. See Pediatric Hemostasis References section in Coagulation Guidelines for Specimen Handling and Procssing in Special Instructions.
- Interferences and Limitations
Cautions: Alpha-2 plasmin inhibitor results are potentially affected by:
- Heparin, unfractionated or low-molecular-weight >4 U/mL
- Alpha-2-macroglobulin >7 mg/mL; potentially leading to a falsely-increased result
- Hemoglobin >200 mg/dL
- Bilirubin >20 mg/dL
- Triglycerides >1000 mg/dL
Double-centrifuged specimen is critical for accurate results as platelet contamination may cause spurious results.

Outside Laboratories: Centrifuge sample and transfer plasma to a separate plastic vial. Centrifuge plasma for another 10 minutes and decant plasma to a separate vial, leaving 0.25 mL in the bottom of centrifuged vial. Freeze platelet-poor plasma at -20°C while awaiting shipment. Transport samples on dry ice.
Stability: Frozen (required): 14 days; Refrigerated: Unacceptable; Ambient: Unacceptable.
Reject Due To: Gross hemolysis, gross lipemia, gross icterus.

Processing
- Receiving Instructions
Promptly forward specimen to the Coagulation bench for processing. Sample must be processed within 4 hours of collection.
Coagulation Bench: Double-spin the sample and aliquot 1 mL (0.5 mL min) plasma into a separate vial. Freeze at -20°C.
- UW-MT: Place in the Sendouts Mayo Frozen rack.
- HMC: Place in the frozen rack for UWMC Reference Sendouts (to be sent Cross Hospital).
