Description

Clinical Utility: The CMV inSIGHT™ T Cell Immunity Panel measures the strength of T cell responses to Cytomegalovirus (CMV) specific antigens. It evaluates and reports the activity of CD4 and CD8 T cell responses independently. Effective T cell immunity against CMV is a factor in controlling CMV viral latency. CMV can affect patients with weakened immune systems and is a common risk factor in patients following solid organ or hematopoietic stem cell transplant.

Approval: Laboratory Medicine Resident (LMR) or Micro Fellow approval is required.

***Collection Note: Samples must be shipped to Eurofins the same day as collection. Blood draws should be appropriately timed to ensure arrival at UW-MT SPS by 13:00 for same-day shipment. Highly recommend collection at UW-MT to eliminate delays. Samples collected at at UW-NW or HMC should be collected prior to 10:00 a.m. with sample in lab by 11:00 a.m. Testing cannot be guaranteed for specimens received after that time.***

Interpretation

Method

Whole blood is stimulated with SEB, CMV antigens, or left unstimulated as a negative control and incubated at 37ºC. During the incubation Brefeldin A is added, causing the interferon (IFN)-gamma to be retained inside the cell. Following the stimulation phase, cells are recovered, stained for surface markers (CD45, CD3,CD4, CD8, CD69) and intracellular IFN-gamma, and analyzed by flow cytometry. This test has not been cleared or approved for diagnostic use by the U.S. Food and Drug Administration.

Collection Timing: Samples must be sent to Eurofins the same day as collection. Time blood collections to ensure that samples arrive at UW-MT SPS by 13:00 for same-day shipment.

  • Collect Monday - Friday only.
  • Do not collect prior to a holiday.

Collection Details: 10 mL blood in GREEN TOP (Sodium Heparin) tube

  • 10 mL tube must be 3/4 full to maintain proper ratio of blood to anticoagulant
  • Accepted alternatives:
    • 4 mL sodium heparin tube completely full
    • 6 mL sodium heparin tube 3/4 full

Phlebotomy: Affix a "Special Handling" label to the specimen bag to indicate expedited processing in SPS.

Outside Laboratories: Due to the limited stability for these samples, outside facilities should arrange for testing directly with Eurofins Viracor.

Processing

Transport and store whole blood at ambient temperature.

  • UW-MT SPS: Notify Sendouts immediately upon sample receipt.
  • Offsite Clinics: Expedite transport to UW-MT Sendouts. Affix a "Special Handling" label to the outside of the package. Send as soon as possible via DLMP courier and notify UW-MT SPS of an incoming sample that requires urgent processing.
    • Samples may also be sent via other approved courier services (Delivery Express, Alpha) if the DLMP courier schedule does not allow for timely delivery.

Sendouts:

  • Order electronically in the Retriever system using Eurofins Viracor Test: 30360.
  • Send samples Monday - Friday only the same day as collection. Samples must arrive at Eurofins Viracor for setup the next day.
  • Package samples in the provided Eurofins Viracor Live Cell shipping kits. Affix the FedEx label to the outside of the box.
  • A dedicated Eurofins Viracor courier will pick up packages at the UW-MT lab window at approximately 3 PM.
  • Samples that miss the daily courier pickup must be sent via regular Priority Overnight FedEx shipment. (If shipping on a Friday, select Saturday Delivery.)

Stability: Ambient (Required): 32 hours; Refrigerated: Unacceptable; Frozen: Unacceptable.

Causes for Rejection: Whole blood received after stability (32 hours after collection), whole blood received cold or frozen, and tubes received less than ¾ full are not accepted and are cause for rejection.
