Description

Useful For: Detecting clinically significant lead exposure in 24-hour specimens. This test is not a substitute for blood lead screening.

Note: If a random urine specimen is submitted, the Lead/Creatinine Ratio, Random Urine test will be performed and results will be reported under Reference Laboratory Test 1 (SEND1).

Interpretation:

Measurements of urinary lead (Pb) levels have been used to assess lead exposure. However, like lead blood, urinary lead excretion mainly reflects recent exposure and thus shares many of the same limitations for assessing Pb body burden or long-term exposure.¹

Urinary lead concentration increases exponentially with blood lead and can exhibit relatively high intra-individual variability, even at similar blood lead concentrations.¹

Patient Preparation: High concentrations of gadolinium and iodine are known to interfere with most metals tests. If either gadolinium- or iodine-containing contrast media has been administered, a specimen should not be collected for 96 hours.

24 Hour Urine (Preferred):

Random Urine: Collect random urine in a sterile screw-cap container (with no metal cap or glued insert).

Handling Instructions

Outside Laboratories:

  • Mix specimen well. Measure and record total volume. Note collection interval and total volume on requisition.
  • Aliquot urine into a Mayo metal-free screw-capped vial (T173)* and refrigerate.

*Note: Mayo T173 is recommended, but not required.

Stability: Refrigerated (preferred): 28 days; Ambient: 28 days; Frozen: 28 days.

SPS: Specimen must be refrigerated within 4 hours of completion of 24-hour collection.

  • 24-Hour Collection: Mix specimen well. Measure and record total volume. Aliquot urine into a Mayo metal-free screw-capped vial (T173)* and refrigerate.
  • Random Collection: Aliquot urine into a Mayo metal-free screw-capped vial (T173)* and refrigerate.

*Note: Mayo T173 is recommended, but not required.

  • 24-Hour Collection: Order Mayo Test: PBU.
  • Random Collection: Order Mayo Test: PBUCR (Lead/Creatinine Ratio, Rand Urine).
    • Batch RUPB on the interface. Add test PBUCR and delete test PBU in MayoAccess prior to closing the batch.
  • Interfaced: Yes.
